Sponsorship Request Form: Use this form to request league sponsorships. We have larger packages available than the standard form found on the PYFL website.

Sponsorship Reciept: The information is identical to the Sponsor Request, however, there is room at the bottom for the business to use as a "receipt" for their paperwork.

Raffle Ticket information: Use this form for the Buckeyes fundraiser raffle ONLY! Remember, the bigger and better the prizes, the more ticket sales we will have!!
